SeaStar’s Hospital to Home Package is designed to provide seamless support during the transition period when returning home from the hospital. This service focuses on comfort, safety, and continuity of care. This package includes:
Care Management Services (3 hours)
Delivered by a licensed Master of Social Work (MSW), these services focus on managing post-hospital care effectively:
- Hospital Coordination: The MSW will meet you or your loved one at the hospital to go over the discharge plan with the hospital staff and the client.
- Safe Transition Home: Providing assistance with transportation from the hospital to the home as well as helping to fulfill any new medication prescriptions.
- Follow-Up Support: Scheduling follow-up medical appointments and arranging transportation.
- Home Health Assistance: Coordination with home health services and durable medical equipment (DME) orders.
- Home Safety Inspection: Conducting a thorough home assessment and providing actionable safety recommendations as well as referrals to trusted resources.
In-Home Care Services (8 hours)
Experienced caregivers offer personalized support to make daily life more manageable and enjoyable:
- Companionship: Providing emotional support and social interaction.
- Personal Care: Assistance with bathing, grooming, and hygiene.
- Light Housekeeping: Maintaining a clean and organized living environment.
- Meal Preparation: Cooking nutritious meals tailored to the client's preferences.
- Errands and Shopping: Picking up groceries, medications, and essentials.
- Medication Assistance: Offering gentle reminders and help with taking medication at the scheduled times.

- Life Alert: One month of life alert service at a reduced rate.
- Prepared Meals: One week of meals for two people or two weeks of meals for one person, delivered at a discounted rate.
